Ø All accidents, incidents and near misses must be reported to UBS via sh-fireandlifesafety@ubs.com
Ø
Anyone
requiring access to the 5 Broadgate roof area must complete the UBS roof
attestation form. This requested via
sh-fireandlifesafety@ubs.com
Ø Suitable ear protection must be worn in all areas where the level of noise has been identified as a hazard (warning signs displayed). Noise risk assessments are available to review upon request.
Ø Any penetration of fire compartments (walls, ceilings and floors) for tasks such as new cable runs, must be fire stopped by a competent/approved contractor (pink expansion foam is NOT to be used).
Ø Walkways and fire escape routes should be kept clear at all times.
Ø All work areas should be thoroughly cleaned on completion of work and no waste is to be left on site.

Ø Remember to give four working days' notice for submission of permits.
Ø In an emergency, call security on 020 7567 2222.
Ø 5 Broadgate Contractor Entrance opening hours are Monday-Friday 07:00-19:00.
Health & Safety Induction: All operatives/site attendees who are listed on Access Permits MUST undertake our H&S Induction. This involves watching our H&S Induction video, correctly answering the questions given and updating your profile accordingly (full details of actions required are provided in the e-Permits User Guide 1 (contractors guide). The H&S induction can be found on the e-Permits log in page.
Ø Access Request Proforma: All RAMS are to be accompanied with a completed ‘Access Request Proforma’. Please speak to your work sponsor if you have any questions in regards this requirement.
